DESCRIPTION:2021 Hostess Volunteer Application for Conference Registration for hostesses and volunteers \n2021 Aplicación de Voluntarios de Becas para Conferencia \n                “Enhancing Our Minds…In 2021”  on  June 23-27\, 2021 \nlocated at the Embassy Suites Orlando-Lake Buena Vista South\, 4955 Kyngs Heath Rd\, Kissimmee\, 34746 \nVisit Embassy Suites CLEAN STAY Program – A CLEANER STAY FROM CHECK-IN TO CHECK-OUT \nApproved applicants applying for a scholarship will be required to volunteer 3 hours. \nVolunteer Qualifications/Requirements: \n\nBe a current provider member of FFCCHA (keep valid thru June 30) operating as a FL registered or licensed provider\n\n—-Or be a current Co-provider member of FFCCHA (keep valid thru June 30) \n\nComplete and submit this scholarship form along with the $35.00 fee–NO Refunds. Returned checks assessed a $47\nAttend a volunteer orientation before going to conference: check only one\n\n       Saturday\, April 17 (English and Spanish orientations) at the Full Board meeting\, select your own volunteer times – only at this virtual meeting \n       May TBA (both in English) at 7:00 PM via phone conference call or Virtual meeting (details emailed to you in May) \n       May TBA in Spanish only at 7:00 PM via phone conference call or Virtual meeting (details emailed to you in May) \n\nAttend the entire conference day(s) that you applied for. Turn in a completed conference\nWear a white blouse with black skirt or pants when serving your 3 volunteer hours. (no tank tops or shorts)\nSign-in at conference and fulfill your 3 hour volunteer duties as assigned\, return hostess paper\, and sign–out.\n\nOnly completed forms submitted with a $35.00 check or money order made payable to FFCCHA will be accepted. \nMail to: FFCCHA Volunteers\, Robin Dean\, 3709 Cabasa Loop\, Valrico\, FL 33596 \nDeadline: April 30\, 2021 Qualifying applications will be dated as received and selected on a first come\, first serve basis until all scholarships have been awarded. Ser Co-proveedora con membresia vijente de FFCCHA (estar valida hasta Junio 30\, 2021)\nCompletar y someter esta solicitud de beca junto con $35. NO devoluciones. Los cheques que sean devueltos por el banco tendráun cobro  adicional de $47.\nAsisitir a una orientación de voluntarios antes de asisitir a la conferencia: marque una sola\n\n\nSábado\, Abril 17 (orientaciónen Inglés y Español) en la reunión general ZOOM\, sólo en esta reunión puede seleccionar sus propias horas para servir como voluntario.\nMayo TBA (las dos en Inglés) a las 7:00 vía telefónica (se le enviará por correo electrónico en Mayo toda la información necesaria para esta conferencia telefónica).\nMayo TBA en Español solamente a las 7:00 PM via telefónica (detalles serán enviadas en Mayo via correo electrónico).\n\n\n Asisitir a la conferencia todos los días que usted haya aplicado. Entregando la evaluación completa de la conferencia.\nVestir con blusa blanca y falda o pantalones negros cuando sirva sus 3 horas voluntarias.
