DESCRIPTION:2022 Hostess Volunteer Application for Conference Registration for hostesses and volunteers \n2022 Aplicación de Voluntarios de Becas para Conferencia \n                “Enhancing Our Minds…In 2022”  on  June 22-26\, 2022 \nlocated at the Sheraton Sand Key Resort\, 2260 Gulf Boulevard.  Clearwater Beach\, FL  33767 \nApproved applicants applying for a scholarship will be required to volunteer 3 hours. \nVolunteer Qualifications/Requirements: \n\nBe a current provider member of FFCCHA (keep valid thru June 30) operating as a FL registered or licensed provider\n\n—-Or be a current Co-provider member of FFCCHA (keep valid thru June 30) \n\nComplete and submit this scholarship form along with the $35.00 fee–NO Refunds. Returned checks assessed a $47\nAttend a volunteer orientation before going to conference: check only one\n\n       Saturday\, April TBA (English and Spanish orientations) at the Full Board meeting\, select your own volunteer times – only at this virtual meeting \n       May TBA (both in English) at 7:00 PM via phone conference call or Virtual meeting (details emailed to you in May) \n       May TBA in Spanish only at 7:00 PM via phone conference call or Virtual meeting (details emailed to you in May) \n\nAttend the entire conference day(s) that you applied for.
